Dr. Azhar obtained his MD (UKM), M. Psych. Med (Malaya) and Post Graduate Diploma in Cognitive Therapy (University of Oxford). He also obtained membership of the United Kingdom Council of Psychotherapists.

Dr. Azhar was attached with Science University of Malaysia, Kubang Kerian as Consultant Psychiatrist from 1988 to 2001 cum lecturer in Psychiatry from 1988 to 1994, Head Department of Psychiatry form 1991 to 1994 and Associate Professor of Psychiatry form 1994 to 2001. In 2001, he's a Professor, Unit of Psychiatry, Faculty of Medicine & Health Sciences and as Deputy Dean in Academic & Clinical, Faculty of Medicine with Putra University of Malaysia and as Senior Consultant Psychiatrist & Psychotherapist, UPM Clinic Kuala Lumpur. He became Dean of the same faculty in 2002 until he retired in 2014.

Zain berkata, histeria merupakan salah satu daripada penyakit yang gejalanya adalah fizikal atau somatik atau lebih dikenali sebagai gangguan konversi (conversion disorder). Punca histeria terdiri daripada beberapa faktor yang berlaku secara serentak. Biasanya ada faktor fizikal yang mempengaruhi seperti hipometabolisme hemisfera dominan dan hipermetabolisme hemisfera bukan dominan otak yang mengakibatkan gangguan komunikasi pada organ itu. Jika terdapat faktor stres yang kuat dan kronik serta berpanjangan berlaku pada masa sama, ia boleh mencetus sindrom gangguan konversi atau histeria," jelasnya ketika dihubungi kru Jurnal baru-baru ini.

According to Dr Azhar, a support system for the elderly is currently lacking in Malaysia, hence it is important for the elderly to take steps to overcome loneliness proactively. As people age, their social situation changes and this is one of the reasons why they feel lonely.
